Disability Information and Access Line

The Disability Information and Access Line (DIAL) helps people with disabilities get connected to information about local community resources that support independent living. DIAL provides information about essential services such as transportation, housing support, disability rights, and more.

NY Connects provides a searchable database of long-term services and supports that can be browsed by category or keyword.

Help Hub for Families

The NYS Council on Children and Families offers this database to find health, education, human services, and disability information for newborns through age 22. Explore their educational resources, locate available services and programs in your area, search for additional online resources, and connect with New York State partners and other organizations.

Housing and Support Services – Search By Region

Housing and Support Services (HSS) under NYS OTDA administers an array of programs to address the problems of homelessness in the State. These programs provide a continuum of services for homeless, at risk and low-income households. Use the map on this page to see a list of providers by region.

Social Care Networks (SCN)

New York State has established nine Social Care Network Lead Entities who are responsible for managing a network of regional partners to provide health-related social needs services for participants of the 1115 Medicaid Waiver.
